AFL care demonstration projects are required to use end of year templates for reporting project progress and performance measure data to the OAPP each year. Below you will find the report templates as well as the instructions for each template.

Adolescent Family Life (AFL) Continuation Applications
AFL grantees must submit a non-compete continuation application each year for the life of their grants in order to be awarded subsequent funding. Please download the OAPP letter regarding dates and requirements: (pdf, 34 kb)
