Much of this application is automatically pre-filled. Enter data in the areas of the application highlighted in yellow.

The SF 424 requests information about the applicant and the project. Fill in the appropriate questions and click “save and continue” at the bottom of the page. If you need to leave the form to you can return later, be sure to click "save this page".

Section A: concerns Non-Profit Status. Check the box if appropriate and remember to attach Exhibit A.

Complete applicant certifications for Exhibit B. By clicking on the individual links you can fill out and print these forms for a signature. Form LLL is only required from applicants that employ lobbyists in an effort to obtain a grant.

Enter Site Information, Population Information, and an Equipment Request for each station in the project.

The forms will automatically fill with much of the information. If any of the information is incorrect, you can make corrections. Please note why any corrections were made in the text box following question 12.
WHAT YOU WILL NEED BEFORE YOU START
Applicant DUNS number All applicants must provide a Dun and Bradstreet Data Universal Numbering System (DUNS) number when applying for Federal grants. Organizations can receive a DUNS number at no cost by calling the dedicated toll-free DUNS Number request line 1-866-705-5711 or via the Internet (fedgov.dnb.com/webform).
Employer/Taxpayer ID Number The Employer's Identification Number (EIN) assigned by the Internal Revenue Service. (It will be redacted prior to any disclosure to the public.)
Applicant Corporate Information (optional) If the Applicant is a non-profit corporation requesting Priority Reimbursement, submit documents indicating non-profit corporate status, (i.e., Articles of Incorporation, IRS letters, corporation registration, etc.)
The FCC Facility ID number For each station for which you are requesting reimbursement.
FCC Authorizations For each station in the application, you will need copies of the most recent FCC authorizations for the Analog and Digital station. (i.e. License or STA or FCC Form 347 or FCC Form 302)
If you have modified an existing system Attach a copy of the transmission system modification letter filed with the FCC pursuant to Sec. 74.796(b)(5) of the FCC rules.
Paid Invoices For each station in the application, you will need copies of paid invoices for equipment you are requesting reimbursement.
