Denominators for computing 2000–2009 case rates were obtained from the Annual Estimates of the Population by Sex, Race, and Hispanic or Latino Origin for the United States: April 1, 2000, to July 1, 2009 (accessed August 26, 2010).

Case counts for race categories (American Indian or Alaska Native, Asian, Black or African American, Native Hawaiian or Other Pacific Islander, and White) do not include persons of Hispanic ethnicity or multiple race.

Data for all years updated through July 1, 2010.
