How do applicants access the funding opportunity announcement?
To access the announcement and application package:

The application deadline date is March 18, 2011.
Is this the new Funding Opportunity Announcement for Healthy Homes or is this a new round of our existing funding?
This is a new competitive announcement for healthy homes and lead poisoning prevention.
This is not a continuation of the existing Childhood Lead Poisoning and Prevention funding which expires on June 30, 2011.
Do applicants with an existing Healthy Homes program have to wait a year before program implementation?
Applicants with an existing healthy homes program and strategic plan will work with the assigned Project Officer to
ensure that the strategic plan addresses all elements of the Funding Announcement. If this occurs prior to the end
of Year 1, the program implementation phase can begin.
Who is eligible to apply?
Eligible applicants that can apply for this funding opportunity announcement include:
- Federally recognized or state-recognized American Indian/Alaska Native tribal governments
- American Indian/Alaska native tribally designated organizations
- Alaska Native health corporations
- Urban Indian health organizations
- Tribal epidemiology centers
State and local governments or their Bona Fide Agents (this includes the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau)
Political subdivisions of States (in consultation with States)
(Political subdivisions of States must serve a population of 1,000,000 or more and have authority to govern, regulate, deliver, implement and enforce policies or codes related to health or housing issues in the jurisdiction identified in the Needs Section of the application. Upon submission of the application, the applicant shall include proof to verify their status).
Please provide clarification on the following statement:
“Not more than 33% of any cooperative agreement or contract funded through this cooperative agreement
may be used for direct assistance of State Programs. State Programs are restricted in the use of funds
to Program Management, Surveillance, Travel, and Indirect Costs. This restriction does not apply to
States whose local programs are state entities.”
If a state is applying, will the state be allowed to use more than 33% of the money to fund state program staff?
No. If the applicant is a state, no more that 33% of the award can be used for state costs associated with Personnel, Travel, or Indirect Costs. No more than 10% can be used for Indirect Costs.Must a state program recipient pass at least 67% of the money to a local partner?
Yes. At least 67% of the award should be used for contractual or sub-grantee agreements with local programs, nonprofit organizations, or other entities that will allow the program to meet their goals and objectives.How does the restriction apply to States whose local programs are also state entities?
If the applicant is a State whose local programs are also employed through the state, thirty-three percent (33%) of the award can be used for costs of the applicant associated with Personnel, Travel, or Indirect Costs.Sixty-seven percent (67%) of the award can be used for contractual or sub-grantee agreements with local programs (who may also be state entities), nonprofit organizations, or other entities that will allow the applicant to meet their goals and objectives.
Can state programs use the funds for health education, case management and/or environmental management?
The use of funds for state programs is restricted to Program Management, Surveillance, Travel, and Indirect Costs.
What documentation should be submitted to satisfy the Medicaid Provider Certification/Statement requirement?
The applicant should submit a Provider ID or National Provider Identifier. The applicant can also submit evidence
that Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) services are billed to Medicaid or supported
by funding mechanisms other than the CDC cooperative agreement.
If the applicant is not a health entity, this information should be obtained from the entity that will provide the health-related activities outlined in the funding announcement.
What hardware and software is required to support the installation of the Healthy Homes
and Lead Poisoning Surveillance System (HHLPSS)?
The HHLPSS Standard Operating Environment Document lists the requirements and estimated cost.

What is the cost of the Healthy Homes and Lead Poisoning Surveillance System (HHLPSS)?
The HHLPSS Software and installation assistance are provided at no cost. However, there may be costs associated with
the hardware and software required to support the installation of HHLPSS.
What technical support will be provided for HHLPSS and how long will the support be provided?
CDC will provide support for the installation and ongoing support for the HHLPSS Software and
Melissa Data Software indefinitely. The applicant’s IT Staff will need to be available for approximately 3 days
to support the installation, configuration and deployment into your environment. The applicant’s IT staff will
also be responsible for maintaining the supporting software and hardware, and will be required to install regular
system updates (usually quarterly).
Which appendix should we include data sources for the population estimate?
Population estimates and their data sources should be provided by all applicants and placed in an
appendix named “Population Estimate”.
Who should sign the Letter of Intent and Travel Assurance Letter?
The Business Official and/or Authorized Representative should sign.
What should be included in the 25 page limit?
All items listed in the FOA under the Project Narrative Section should be submitted within the 25 page limit.
This includes Need, Program Elements, Year 1 Work Plan, Evaluation, and Project Management Staff.
Should we address Letters of Support to the Program or Grants Management Official?
Letters of support should be addressed to the Program Contact listed in the Funding Opportunity Announcement.
