How can a veteran get his/her DD Form 214 corrected?

You can submit a dated and signed written inquiry or a SF Form 180 (download .PDF form). More information can be obtained at Veteran Record Corrections.

How can a veteran get his retirement points corrected if he/she is discharged?

You can submit a dated and signed written inquiry. Specify which retirement years are in error. Attach supporting documents such as Leave and Earnings Statements (LES), DD Form 214, DA Form 1383 or DA Form 1380, list of unit assignments, and DD Form 220. Send requests to:

How can a veteran obtain a Statement of his/her military Service?

You can submit a dated and signed written inquiry or a SF Form 180 (download .PDF form). Your request should state that you desire a statement of service.

What does the Statement of Service show?

It shows your military history from your date of entry to your date of discharge or retirement. It will include all promotions/ grade reductions and dates of ranks, all active and inactive duty dates, Character of Service and Narrative Reason for Separation for each period.

What does the statement of service with retirement points show?

It shows all points earned during your military career.

How can a veteran obtain his/her Discharge or Retirement Certificate?

You can submit a dated and signed written inquiry. You should state that you desire a discharge or retirement certificate. More information can be obtained at Proof of Service Replacement Documents. Send inquires to:

How can a veteran receive a copy of his/her DD Form 214?

You can submit a dated and signed written inquiry or a SF Form 180 (download .PDF form) to:

What if the veteran receives notification that the request was forwarded to HRC?

Your record may be in HRC's holding area or in the process of being transferred to NARA. HRC attempts to locate the record or contacts other agencies for a copy of your DD form 214 or for certification of your active duty.

What if HRC cannot locate a copy of my DD Form 214?

If you were never issued a DD From 214, you may request issuance. If you lost or misplaced your DD form 214 a replacement transcript or Statement of Service will sent to you. For more information, visit Proof of Service Replacement Documents.

What can be done when a veteran's records were destroyed or lost?

HRC does all it can to verify your service and/or send you a listing of other sources to contact for copies of your separation documents. If you can provide the name and number of your unit, including the battalion, brigade, company, etc., request a Morning Report Search for
dates of active duty from the National Archives and Record Administration.

How can a veteran have his/her name, Social Security number or date of birth changed in his military records?

You can submit a dated and signed written inquiry or a SF Form 180 (download .PDF form). Supporting documentation (such as a divorce decree, court order and birth certificate) should be attached to the request.

If you are a discharged veteran, changes to your official records are only warranted if the change occurred during your tour of active duty or reserve service. More information can be obtained at Veteran Record Corrections.

If I am a veteran, how can I obtain a VA Home Loan?

To obtain a VA Home Loan, you must obtain a Certificate of Eligibility from your local Veterans Administration office. If you already have documents which reflect 90 consecutive days or more or 24 months of Active Duty (AD) or Inactive Active Duty Training (IADT) or six qualifying years as a reservist (DARP Form 249 / AHRC Form 249-2-E / AHRC Form 606-E ), the VA will accept those documents. If you do not currently have those documents, you can submit a dated and signed written inquiry or a SF Form 180 (download .PDF form). The request should specify that you need a statement of service for the purpose of obtaining a VA Home Loan.

How can a veteran find out what benefits he/she is entitled to receive?

HRC can only verify your military service. You can contact your local Veterans Benefits Administration office for a determination of your eligibility to receive benefits. More information can also be found at the Department of Veterans Affairs.

If a veteran never received or lost his medals that he/she was awarded while on active duty, can he/she obtain them now?

If I was a civilian on an Army transport ship during World War II, can that time be converted to military service?

If you served from 7 December 1941 to 31 December 1946 you can submit an Application For Discharge Of Member Or Survivor Of Member Of Group Certified To Have Performed Active Duty With The Armed Forces Of The United States (DD Form 2168) (download .PDF form). Can a veteran receive college credit for the military schools he/she attended?

You can request a transcript of your military schooling by submitting a signed and dated written request or you can submit a SF Form 180 (download .PDF form). When you receive the document, you can take it to the college or university. More information can be obtained at Proof of Service Transcripts.

How can I obtain copies of my medical records?

Contact your local Veterans Health Administration office. It is possible that they will know the location of your medical records.

What can I do if the local Veterans Administration Office cannot assist me?

Copies of your medical records may be located in the National Archives and Records Administration if you were discharged prior to October 1992. To request the records, download the SF Form 180, Request Pertaining to Military Records (download .PDF form).
