Record Update and Maintenance

For Soldiers

Verifying Your OMPF

All Soldiers regardless of component are responsible for updating their individual OMPF.  Soldiers must keep their personnel records updated at all times by routinely reviewing their record online. At a minimum, Soldiers will review the OMPF annually to ensure the record is accurate, free of errors, and representative of all authorized documents (click here for a list of Authorized Documents). You may view your record via AKO or you can visit https://iperms.hrc.army.mil.

Add/Delete/Misfiled Documents

Record Managers (RMs) are authorized to web upload documents for inclusion in the OMPF. Record Managers are defined as Military Personnel Divisions (MPDs), S1 Personnel, Human Resource Personnel, Unit Administrators, or any other individual authorized to service the OMPF regardless of component.  If document(s) need added, deleted or are misfiled in your OMPF or you have questions about your record, contact your RM. IRR and IMA Soldiers are to contact their Career Management Branch for this action. If a document is being added to your OMPF, your RM can provide a Batch Number to you when they submit documents.  The Batch Number is used to track submitted documentation while being processed in iPERMS.

Normally it will take up to three weeks after the document is uploaded by the RM to be viewed in iPERMS. Documents submitted for DA Selection Boards will be processed within three business days into iPERMS.

Update and Corrections of Board Documents

Contact your Records Manager (RM) for adds, updates, corrections, and deletes to your OMPF.  Record Managers are defined as Military Personnel Divisions (MPDs), S1 Personnel, Human Resource Personnel, Unit Administrators, or any other individual authorized to service the OMPF regardless of component.  The updates will be processed into iPERMS within three business days during the board window.

The Performance Folder in iPERMS contains the documents that update your Board File which is maintained by the DA Secretariat. You will not see the updates in My Board File until the DA Secretariat Board Team refreshes the Board file. The DA Secretariat refreshes the file periodically up to the Board convene date.

OERs, NCOERs, or DA Form 1059s

To add OERS, NCOERs, or DA Form 1059s, in iPERMS, submit to HRC via AKO Forms which requires digital signatures and a recent version form.

Sending evaluations by email is only for deployed units or rating officials and should only be used if AKO Forms is not available

All non-deployed units or rating officials without digital signatures must mail printed evals to: CDR, USA HRC, ATTN: AHRC-PDV-ER, 1600 Spearhead Division Ave, Dept# 470, Ft Knox, KY  40122. 

For Deployed units Only adding or correcting OERs, NCOERs, or DA Form 1059s send to:  usarmy.knox.hrc.mbx.tagd-eval-from-deployed@mail.mil.

To correct any discrepancies on OERs, NCOERs, or DA 1059s, contact: usarmy.knox.hrc.mbx.tagd-eval-appeals@mail.mil.

Department of the Army (DA) Photos

DA Photos are maintained in the Department of the Army Photo Management Information System (DAPMIS).  You cannot view your photo in DAPMIS. Your photo can be viewed in iPERMS.

Derogatory Information in the Performance Folder

AR 600-37, Unfavorable Information, Chapter 7, outlines the provisions to allow a Soldier to request derogatory documents be transferred from the Performance folder to the Restricted folder within the OMPF. The document must have been filed for at least one year before a Soldier can request its transfer.

To request derogatory information be removed from a Soldier's OMPF, the Soldier must submit a request to:

For Record Managers (S1s, UAs, MPDs)

Web Upload Access

If you do not have web upload capability, contact the Access Control Team at usarmy.knox.hrc.mbx.iperms-accounts@mail.mil  or 502-613-9109 (DSN 983-9109) for web upload access and information. 

Refer to MILPER 10-333, Clarification to MILPER Message 10-322, Submitting OMPF Documents into the interactive Personnel Electronic Records Management System (iPERMS).  Procedures for requesting Scan Operator (Web Upload) access are in MILPER 11-230, Revised Procedures for Requesting Access to the Official Military Personnel File (OMPF) in the interactive Personnel Electronic Records Management System (iPERMS).

Add/Delete/Misfiled Documents

RMs can email a list of corrections with the document names and effective dates to usarmy.knox.hrc.mbx.perms-records@mail.mil by submitting the Correction Request Form when identifying documents in iPERMS which need to be corrected. Please fill out the form in its entirety. If documents indentified for correction are OERs, NCOERs, or DA Form 1059, send an email to usarmy.knox.hrc.mbx.tagd-eval-appeals@mail.mil. Do not mail personnel records/documents to iPERMS office, the National Personnel Records Center, or the Veteran's Administration, they must be web uploaded.   

iPERMS Support

If you are unable to submit documents to iPERMS, first ensure you have the correct web upload access, Scan Operator (SO) or Field Operator (FO). If you do not have the correct access, contact the Access Control Team at usarmy.knox.hrc.mbx.iperms-accounts@mail.mil or 502-613-9109 (DSN 983-9109).
