Have you ever asked for a copy of your health record and reviewed it? If so, tell us about it! 

We invite you to participate in the "What’s in Your Health Record??" Video Challenge (#YourHealthRecord). Create a short, compelling video (up to 2 minutes in length) sharing how getting access to review your health record and checking the information can help make sure you or your loved ones get the best care. It’s important that you and your health care providers have the right information to make the best decisions.  

Patients and their families have a legal right to request to see and get a copy of their health record from most doctors, hospitals and other health care providers, such as pharmacies and nursing homes, as well as from their health plans.   Patients can use that information to better understand their health, to coordinate their care with other health care providers or to help identify possible errors or omissions in their medical records.  Patients can get access to their health information through a patient portal, or a personal health record (PHR) or by email.  

Your video should share a personal story or experience illustrating how having access to your health record and looking it over helped improve care.  Maybe you reviewed your health record and spotted a missing medication, allergy, or test result.  Or perhaps you are changing doctors and want your new doctor to have your most recent treatment history.  Entries could include scenarios such as (these are just examples — be creative and craft your own or elaborate to inspire others!): 

• I looked at my health record through my doctor’s patient portal and noticed that the list of medications I was taking was out of date.  I notified my doctor that I was no longer taking one of the medications.  He updated the list so that when I was referred to a specialist, he had the most accurate information about my current medications and could make sure that the new medication he was prescribing didn’t interact with others I was on. .  
• My doctor gave me electronic access to a patient portal where I can view my child’s health record and track all her immunizations and doctor visits.  When I recently moved to a new state, it was really helpful to be able to download this information and share it with my new doctor.  Having this information readily accessible is helpful in making sure that my daughter is up-to-date on her vaccines.  

The goal of this video contest is to illustrate specific benefits of having access to view the information in your health record and reviewing that information.  We hope your stories will motivate and inspire others to ask to see their medical record and to use that information to improve their own care or the care of a loved one.  

Check out the FAQs for more information about your legal right to see and get a copy of your medical record, reasons why you may want to ask for a copy, as well as suggestions on what to look for when reviewing your record.  

Note: To participate, you must be a citizen or permanent resident of the United States. Team members must be at least 18, but your video can include minors under 18 as long as a parent or legal guardian signs their consent forms. For details on eligibility, review the Official Rules.

1. Create an account on YourRecord.Challenge.gov or log in with an existing ChallengePost account.
2. On YourRecord.Challenge.gov, click "Accept this challenge" to register your interest in participating. This step ensures that you will receive important challenge updates.
3. Create a video and ensure the following (please read the Official Rules for complete requirements):
1. Your video addresses questions such as:
1. What prompted you to ask for an electronic copy of your health record?
2. What did you find when you reviewed your health record?
3. How did you, or your health care provider, improve your quality of care  after gaining access to your health record? In other words, what was the benefit of being able to view what was in your record? 
4. what did you, or your provider, learn from accessing your health record? Was any information missing or incorrect? 
2. Your video gives a specific example (personal story, experience, testimonial, or thoughtful idea) of the benefits of having access to view your health record and the ability to review what is in your health record
3. Your video encourages viewers to visit www.HealthIT.gov  and to ask their health care provider to see and get a copy of their medical record.
4. Your video is no longer than 2 minutes. In fact, shorter is better, as long as you get the point across!

Confirm that you have read and agreed to the Official Rules. Submit your video by including a link to the video on YouTube.com or Vimeo.com; a text description of how you or a loved one benefitted from having access to your health record. Upload consent forms for everyone who appears in your video, regardless of age.
