To get an original card for your child, you must show us documents proving your child's identity, age and U.S. citizenship. If your child is age 12 or older, he or she must appear in person.
To get a replacement card, you must show us documents proving your child's identity. You must also show us documents proving your child's age and U.S. citizenship, if they are not already in our records.
You must also prove your own identity.

NOTE: While you can use a birth certificate to prove age or citizenship, you cannot use it as proof of identity. Social Security needs evidence that shows the child continues to exist beyond the date of birth. |
