 The Phoenix District Office is open Monday-Friday from 8:00 a.m. to 4:30 p.m. Intake interviews occur Monday, Tuesday, Thursday & Friday from 8:00 a.m. to 3:00 p.m. Please take the assessment found at https://egov.eeoc.gov/eas/ and bring the completed Intake Questionnaire with you to the interview. Limited appointments are available; please call 602-640-5000 for information. If you live outside of the Phoenix Metro Area, or a disability prevents you from coming in person, please mail your Intake Questionnaire to our office.
