Medical units deploy and require a significant number of providers to have TMDS access with personal health information (PHI). PHI access is a separate step requiring verification from a supervisor validating the user has a legitimate reason to access PHI. In order to streamline this, there is a process that allows for PHI access verification for groups of people.

1. List the personnel that require PHI access using the TMDS Bulk PHI Request spreadsheet template and verify each person’s HIPAA certification.
1. For active-duty personnel, answer questions one and two
2. For government civilians, answer questions one through three
3. For contractors, answer questions one through four
2. Submit the completed form to the TMDS Helpdesk.
3. Check the status of PHI requests via the TMDS Helpdesk.
