Regional Training Schedule for events hosted by State Primary Care Associations
Each year health center program grantees report on their performance using the measures defined in the Uniform Data System (UDS). Technical assistance resources to assist grantees collect and submit their data are compiled on this page.
New 2011 Individual Health Center Data and Health Center adjusted quartile ranking are now available on the UDS website. The Health Center adjusted quartile ranking assesses a health center’s clinical performance compared to other health centers. Please click on the following links for more detailed information.

Uniform Data System 2012 Changes
April 18, 2012, 3 – 5 pm ET
Objective: Changes for 2012 UDS include a new Table 5A, Tenure for Health Center Staff; revised Table 6B, Selected Diagnoses and Services; three new clinical performance measures; and questions for Electronic Health Records and quality recognition.
Beginning in calendar year 2012, FQHC Look-Alikes will submit data annually through the Uniform Data System. Technical assistance resources specific to FQHC Look-Alikes are compiled here.
