Thomas C. Slade
Colonel Slade earned a Bachelor of Science degree in
Health Administration from Eastern Michigan University in 1985 He
enlisted in the Army under a college option program and was
commissioned in the Army Medical Service Corps through the Officer
Candidate School in 1987. He began his career as an officer in the
197th Support Battalion, 197th Infantry Brigade (M) (S), Fort Benning, Georgia as a Medical Platoon Leader and Brigade Medical
Supply Officer. He later became the S-4 for the Support Battalion.
COL Slade was assigned to the 2nd Mobile Army Surgical Hospital,
34th Medical Battalion, Fort Benning, as the
Chief of Supply and Services and deployed in this capacity with the
unit in support of Operations Desert Shield and Desert Storm. Upon
return from Desert Storm, COL Slade took over as the Company
Commander in the unit.
After completing a successful command, COL Slade completed the Army
Medical Department Advanced Course and then he attended long term
civilian training at Worcester Polytechnic Institute in Worcester,
Massachusetts where he earned a Master of Science degree in
Technology Management. Upon degree completion he was assigned to the
United States Army Medical Materiel Agency (USAMMA), Fort Detrick,
Maryland. At USAMMA, COL Slade served as the Chief, Equipment
Acquisition Division overseeing acquisition programs to support
equipage and sustainment requirements for the Army Medical
Department (AMEDD) TOE field force.
COL Slade’s next assignment was with 1st Medical Group, Fort Hood,
Texas where he served as the Support Operations Officer for a short
period, prior to being selected as the Executive Officer, 36th
Medical Evacuation Battalion. During his tenure as the XO, COL Slade
deployed with the unit in support of Operation Joint Forge as the
Task Force Medical Eagle XO, providing combat health support
throughout Bosnia, Croatia and Hungary. COL Slade was then assigned
to the 121 General Hospital, Korea as the Chief of Logistics, where
he served for 2 years.
COL Slade was assigned for his second tour at USAMMA as the
Director, Materiel Acquisition Directorate, serving as the principal
acquisition executive for all AMEDD acquisition programs to support
the equipping requirements of the AMEDD. COL Slade obtained his
Level 3 certification in Acquisition Logistics and membership in the
Acquisition Corps. He then served as the Commander, United States
Army Medical Materiel Center – Southwest Asia (USAMMC-SWA) in
support of Operations Iraqi and Enduring Freedom, providing world
class medical materiel support, medical maintenance, and optical
fabrication support as the Single Integrated Medical Logistics
Manager (SIMLM) for the Theater. COL Slade’s then served as the
Executive Officer, 6th Medical Logistics Management Center, Fort
Detrick, MD.
COL Slade commanded the 16th Medical Logistics Battalion at Camp
Carroll, Korea from May 2006 until it’s inactivation in October of
2008 and then established and served as the first Director of the
U.S. Army Medical Materiel Center – Korea (USAMMC-K), providing
world class medical logistics support to the Korean peninsula and
other operations throughout Southeast Asia. COL Slade departed Korea
to attend the U.S. Army War College where he obtained a Masters
Degree in Strategic Studies.
COL Slade’s most recent assignment was serving as the Deputy Chief
of Staff for Logistics, U.S. Army Medical Research and Materiel
Command.
Colonel Slade’s Decorations include the Bronze Star Medal, Army
Meritorious Service Medal with Silver Leaf Cluster, Army
Commendation Medal with 1 Oak Leaf Cluster, Army Achievement Medal
with Silver Leaf Cluster, and the Expert Field Medical Badge. COL
Slade is a member of the Order of Military Medical Merit. |

James E. Chambers
Sergeant Major James E. Chambers entered the U.S.
Army in August 1986 and completed One Station Unit Training at Fort
Benning, GA as an Infantryman (11B). He reclassified and completed
training as a Medical Logistics Specialist (68J) in June 1991 at
Fort Sam Houston, TX.
His assignments include: 1/27 Infantry Battalion (Wolfhounds), 25th
Infantry Division Light at Schofield Barracks, HI; 47th MEDSOM at
Fort Hood, TX; 147th MEDLOG BN (Rear) at Fort Sam Houston, TX;
Tripler Army Medical Center in HI; HHC, 46th Corps Support Group (ABN)
at Fort Bragg, NC; B, 32nd MEDLOG BN (FWD) at Fort Bragg, NC; San
Antonio Recruiting Battalion and 5th Medical Recruiting Battalion at
Fort Sam Houston, TX; 551st Medical Company (Logistics) at Fort
Lewis, WA; USA Medical Research and Materiel Command, Fort Detrick,
MD; USA Sergeants Major Academy at Fort Bliss, TX; and he is
currently serving as the Sergeant Major for USA Medical Materiel
Center, Europe (USAMMCE) in Pirmasens, Germany.
His previous leadership positions include Property Management NCOIC,
Platoon Sergeant, First Sergeant, and NCOIC for the Deputy Chief of
Staff for Logistics, and Sergeant Major.
SGM Chambers’ military education include: Primary Leadership Develop
Course, Basic Noncommissioned Officer Course, Advanced
Noncommissioned Officers Course, Army Recruiter Course, Health Care
Recruiter Course, First Sergeant Course, and the USA Sergeants Major
Course – Class 62. He also earned an Associates Degree in Business
Management Administration, a Bachelor of Science – Liberal Arts, and
a Master of Science in Information Technology Management.
His awards include: Bronze Star Medal, Meritorious Service Medal (3
OLC), Army Commendation Medal (4 OLC), Army Achievement Medal (3 OLC),
Army Good Conduct Medal (8th Award), National Defense Service Ribbon
(2), Iraq Campaign Medal (2 CS), Global War on Terrorism Service
Medal, NCO Professional Development Ribbon with numeral 4, Army
Service Ribbon, Overseas Service Ribbon with numeral 3, the Expert
Field Medical Badge, the Air Assault Badge, the Drivers Badge, the
coveted Recruiter Ring, and the Order of Military Medical Merit. |
