Model for Treating Tobacco Use and Dependence

The model is a flow chart beginning with general population: patient presents to a health care setting (clinic, hospital, work site, others): ask—screen all patients for tobacco use, chapter 2. For never users, go to primary prevention and return to general population. For former users, go to prevent relapse, chapter 3C and return to general population.

For current users, advise to quit, chapter 3A, then assess willingness to quit, chapter 3A. If the patient is willing to quit, assist with quitting, chapters 3A and 4, then arrange followup, chapter 3A. For a relapse, return to advise to quit, chapter 3A. For patients who remain abstinent, go to prevent relapse, chapter 3C, and return to general population.

If after assessing willingness to quit, chapter 3A. the patient is not willing to quit, promote motivation to quit, chapter 3B. If the patient remains unwilling, return to general population. For patients now willing to quit, assist with quitting, chapters 3A and 4, then arrange followup, chapter 3A. For a relapse, return to advise to quit, chapter 3A. For patients who remain abstinent, go to prevent relapse, chapter 3C and return to general population.
