Reminder Postcards Have Replaced Renewal Forms On January 1, 2009, the Dental Board stopped mailing renewal forms and began mailing a 5" x 8" reminder postcard to all licensees. This postcard is mailed approximately 45 days prior to the expiration date of the licensee's registration certificate. Notify the Dental Board of any change of address within 60 days of a move. Address changes are accepted by e-mail, fax or ground mail. A Change of Address Form is available here. If you e-mail or fax your new information, provide your name, profession, license or registration number, phone number, and your new primary mailing address (home or office).
