Applying for a license online is convenient and easy, requiring only a few simple steps:

• Enter all requested information.
• Review the information you entered and modify, if necessary.
• Pay the licensing fee(s) using one of the following:
  • MasterCard,
  • Visa,
  • Discover,
  • American Express,
  • Electronic Check, or
  • Third Party Pay
• View and print receipt.

Be sure to review the "Additional Information Required" section of the Receipt page so you'll know what to do after completing this online application.

Please Note: If you intend to claim a fee exemption because you exclusively provide EMS care for a licensed EMS firm, without receiving remuneration for your services, then you are not eligible to apply online. Please download and print out the paper certification/licensure application from the agency's website.
