Welcome to the Food, Drug, Device, and Salvage Licensing Application

Applying for a license online is convenient and easy, requiring only a few simple steps:

  • Enter all requested information regarding your place of business and ownership structure.
  • Review the information you entered and modify, if necessary.
  • Pay the licensing fee(s) using one of the following:
    • MasterCard,
    • Visa,
    • Discover,
    • American Express, or
    • Electronic Check.
  • View and print the receipt. Be sure to review the "Additional Information Required" section of the Receipt page so you'll know what to do after completing this online application.

Please note: If you intend to claim a fee exemption for either of the reasons listed below, you are not eligible to apply online. Please go to the DSHS Food and Drug Licensing website or call 512.834.6626 to request the correct application. These exemptions may apply to food salvage, drug salvage, device salvage, device distributor, device manufacturer and drug distributor only.

  • Your place of business is a charitable organization as described in the Internal Revenue code of 1986, 501(c)(3), or a nonprofit affiliate of the organization, to the extent otherwise permitted by law. This, however, does not apply to a food manufacturer, food wholesaler, warehouse operator, drug manufacturer or out-of-state drug distributor.
  • This place of business engages only in the manufacture or distribution of radiation machines which are devices or the manufacture or distribution of devices which contain radioactive materials, and is currently licensed or registered with the Texas Department of State Health Services, Radiation Control under 289.252 or 289.226,25 Texas administrative Code.
  • Wholesale Distributor of Drugs engaged only in the distribution of an over-the-counter drug by a charitable organization, as described in the Internal Revenue Code of 1986, 501(c)(3), to a nonprofit affiliate of the organization to the extent otherwise permitted by law.
