When submitting an application for registration as a Sanitarian, please attach the following:
     1.  Job description of past and present work experiences relating to sanitarian activities, including the employment dates of each work experience.
     2.  CERTIFIED college transcript(s) from all colleges/universities (original).      
     3.  Small photograph.
     4.  Registration fee.
     5.  Application must be notarized.
     6.  If currently certified by NEHA as REHS/RS, attach proof of current certification

Please type or print the information requested in all spaces of the application as it becomes your permanent registration record. Complete information in your application is necessary to evaluate your qualifications.
