Release Forms
Authorization for Release of Information Form 1A004
This form must be used when a custodial or noncustodial parent wishes to authorize the OAG to:
- release case status information
- redirect correspondence
- redirect payments
to another person, company, organization, or governmental entity.
Revocation of Authorization for Release of Information Form 1A005
This form must be used when a custodial or noncustodial parent wishes to cancel their authorization for the OAG to:
- release case status information
- redirect correspondence
- redirect payments
to another person, company, organization, or governmental entity previously named on the Form 1A004.
Direct Deposit
Child support payments you receive from the Texas Child Support Disbursement Unit may be sent to you by direct deposit. Direct deposit is a safe and convenient method of receiving your child support.
HOW DOES DIRECT DEPOSIT WORK?Now you can also have your payments directly deposited to your Texas Debit Card.
The Child Support Division offers direct deposit for those child support recipients who have bank accounts. Instead of printing and mailing a child support check to you, the Child Support Division can electronically deposit your money into your bank account. Your child support payments are protected and you avoid delays that can occur through the mail.
HOW TO APPLYIf you want the payments you receive from the Texas Child Support Disbursement Unit to be directly deposited into your bank account, complete and return the Direct Deposit Authorization Form to:

National Medical Support Notice
The National Medical Support Notice is a qualified medical support order and serves notice that the employee identified on the document is obligated by a court or administrative child support order to provide health care coverage for the child(ren) identified in it.
Request for Review of
National Medical Support Notice
A Request for Review of National Medical Support Notice (NMSN) may be used by an employee to contest withholding based upon a mistake of fact, provided the request is received by the OAG within 75 business days after receipt of the NMSN by the employer.
