The Incident Investigation Program responds to radiological incident reports, complaints, technical assistance requests, and close-out requests.

Incidents
Loss of control of sources of radiation that causes, or threatens to cause, or creates a hazard to health and/or environment. View incident summaries.

Complaints
Allegations of unsafe or perceived unsafe conditions involving radiation.

Technical Assistance
Assistance provided to members of the public upon request to address radiological concerns.

Close-Outs
Verifications that facilities vacated by users of radioactive materials are not contaminated.

Our goal is to help ensure a licensee or registrant is taking appropriate steps to protect workers, the public, property, and the environment.
