The Department of Aging and Disability Services (DADS) offers the information in this website to help you evaluate the quality of long term care services. You can use QRS to obtain specific information about a particular long term care provider or to compare providers in a particular area. QRS provides information that can help you identify providers that may meet your family member's needs, but it is not meant to serve as the only basis for choosing a particular provider.
After using QRS to identify providers that look promising, do your own first-hand
inspections and evaluations. Discuss your alternatives with a doctor or other treatment professional
who knows your needs and those of your family and who is also familiar with the providers
of services. The information that QRS provides is not a substitute for visiting
with prospective providers, talking with their staff and visiting with their
clients in order to help you make an informed choice.

Because QRS shows information from a limited time period and is updated only once each month, it may not include some important events that are either older or more recent.
