Guidelines

In order to complete the online application the following information will be requested:

  • Social Security number
  • Drivers License or State-Issued Identification Number
  • Contact Information
  • Information regarding Professional Credentials
  • Information regarding Education and Training

If you do not have a Social Security number, you will be unable to complete this application process, either on-line or by mail. In accordance with the requirements of the Texas Family Code, Section 231.302, you must have a Social Security number in order to hold a license or certificate issued by a state agency. Please visit your local Social Security office to obtain a social security number.

Privacy Notification:

With few exceptions, you have the right to request and be informed about information that the State of Texas collects about you. You are entitled to receive and review the information upon request. You also have the right to ask the state agency to correct any information that is determined to be incorrect.
