FY 2009 Contract Closeout

Closeout packet, with original signatures, and all other required reports are due no later than 60 days after the contract end date for all sub-recipient contracts. For contracts ending August 31, 2009, closeout packets are due October 31, 2009. For contracts ending September 30, 2009, closeout packets are due November 30, 2009.

For all contracts, RFAs/invoices/billings must be submitted through the BHIPS/electronic clinical management system or alternate method as approved, no later than 60 days after contract end date. RFAs/Invoices/billings received after 60 days may not be paid by DSHS.
