| Tariffs and Price Sheets |
|
CLEC (COA & SPCOA), IXCs, PPCs and Other Nondominant CTUs
Shall file either a Tariff or Price Sheet before service begins. There is no approval process and there is no required format. The Tariff or Price Sheet becomes active the day it is filed with Central Records. The Tariffs or Price Sheets shall be filed using Control Number 27385.
ILEC (CCNs)
Shall file General Tariffs and Informational Tariffs using the filing format required by Substantive Rules 26.207 and 26.208. General Tariffs have a 35-day approval process and Informational Tariffs have a 10-day approval process. Each Tariff filing will receive a new docket number, which will be assigned by Central Records.

Frequency: Annually
Due Date: On or before December 30th, each telecommunications utility shall file an Annual Progress Report on Workforce Diversity for the previous four quarters ending on September 30th.
A copy of the Annual Progress Report shall be sent to all Legislative Contacts for Workforce Diversity Reporting.
Filing of Reports:
Annual Reports shall be filed in Project No. 30240
