Caremark Mail Order

HealthSelect^SM of Texas participants can register with Caremark, the new Pharmacy Benefits Manager, on the Caremark website. This enables participants to take advantage of Caremark's many services, including online prescription refills. Use the follow steps to use the mail order service form:

1. Register on the Caremark website (need your prescription card).
2. Once registered and logged in, click the Prescriptions & Coverage link.
3. Use the New Prescription link on the left side of the page.
4. Open the Mail Service Order Form.

Parts of the form pre-fill with the your personal information. Complete the additional fields, print, sign, and mail the form to the address provided with a copy of your prescription. Your prescription must be written by your doctor for a 90-day supply of your medication with three refills.
