HumanaDental DHMO

In order to access benefits under HumanaDental DHMO, you must select and use a primary care dentist (PCD). You will not be able to see a dentist until you have chosen a PCD with HumanaDental DHMO. To select a PCD, call HumanaDental Customer Service toll-free at (877) 377-0987.

To view a list of participating dentists, you can use the DHMO Dentist Finder. After you enter your location, be sure to choose "Dentistry" only from the Search Criteria box, so your results include only dentists you can choose as your PCD.

If you need to see the dentist in October, make sure you call HumanaDental and select a PCD by September 15. Any PCD selections made after September 15 will not be effective until November 1.

State of Texas Dental Choice Plan

With the Dental Choice Plan, you can use any dentist; however, you will get higher benefits by using a dentist in the HumanaDental PPO network. To see a list of dentists participating in the network, use the PPO Dentist Finder.

Annual maximums

Each covered individual has a $1,500 annual maximum, and annual maximums are per calendar year. The $1,500 annual maximum with HumanaDental is an increase from $1,250 under the prior plan. Any benefits used toward prior plan annual maximums carry forward to the new plan. For example:

• If you used $1,000 in benefits under the prior plan, you will have $500 in benefits under the new plan for the rest of 2009.
• If you reached your $1,250 annual maximum under the prior plan, you will have an additional $250 in benefits under the new plan for the rest of 2009.
• If you reach the annual maximum with Humana, charges are your responsibility until the annual maximum starts over at the beginning of the calendar year.

Deductibles

All deductibles are per calendar year. Your prior plan will forward to HumanaDental any amounts paid toward your annual deductible.
