Applying to Become an Dental Board-Approved Dental Assistant Course ProviderA. Course Provider Qualifications: Qualified organizations interested in preparing and submitted a course for approval must meet all criteria outlined in SBDE Rules 114.21, 114.22 and 114.23 (effective May 29, 2006) which can be reviewed and downloaded below. The following entities are authorized to prepare and submit courses for review: 1. Dental, Dental Hygiene, or Dental Assisting Programs that are accredited by the American Dental Association Commission on Dental Accreditation. Visit www.ada.org to learn more about becoming CODA accredited. OR 2. A dental industry professional association.
B. Dental Assistant Course Implementation Date Criteria: C. Course and Examination Approval Consideration by the SBDE: Courses are being accepted for consideration and approval by the SBDE at this time. A provider seeking approval of a dental assistant course must submit the following materials to the SBDE and receive written authorization from the SBDE prior to offering a course: 1. A complete, signed, and notarized Dental Assistant Course Provider Application, as promulgated by the SBDE. 2. An application fee of $100 payable by check or money order made payable to the State Board of Dental Examiners; and 3. All documentation pertaining to the course as outlined in Rule 114.21.
D. Notification of Final Approval: Applicants will be notified of course approval by U.S. Mail along with a request for the following information prior to offering the course as outlined in Rule 114.21. Changes to Published Course Schedules, Curriculum or Materials: Changes must be sent in writing to the SBDE prior to their implementation in the course either by U.S. Mail, e-mail, or fax to: Texas State Board of Dental Examiners |
