Health and Human Service Providers

Health and Human services providers are eligible for inclusion in the statewide information and referral network.  

First, if your program is already listed in the resource database for your local Area Information Center, then no further action is necessary.  

To request to be added to the statewide database, please read these instructions, and then submit the online form or download and mail the pdf version.  

Please contact your closest Area Information Center if you have questions about getting listed in the statewide network.  

State Agency Health and Human Services

State agency health and human services programs are eligible for inclusion in the Search for services.
