To obtain a certified copy of an original application for a Texas driver license or identification card, submit your request on the DR-1 form along with the $1.00 fee. Make sure to complete all blanks and write "Copy of Original Application" in the record request box.

To obtain a certified abstract of an operating record, complete and submit the SR-36 form along with the $20.00 required fee.
Privacy Act requires that the DR-1 or SR-36 form be completed before record information can be released.
You may apply for a change of address for a Texas Driver License or Identification Card by mail, using the DL-64 form. Submit the form and the $10.00 required fee to Texas Department of Public Safety, PO Box 15999, Austin, Texas 78761-5999.
