Note 1: Form ETA-750A is two-sided and must be submitted two-sided. You may either submit the form in a double-sided manner by doing one of the following:

a. set your printer to duplex mode and print it double-sided, or
b. print it single-sided and use a photocopier to combine the pages.

Note 2: The form ETA-750 will no longer be used for the H-2A program on and after January 17, 2009 and for the H-2B program on and after January 18, 2009.
