I am a doctor/nurse and will be reporting to NMCP soon, how do I contact the Professional Affairs/Credentialing department?

The NMCP Professional Affairs/Credentialing department has changed it's name to Medical Staff Services Office.

How do I Obtain a Birth/Death Certificate?

NMC Portsmouth does not issue Birth/Death Certificates.  You will have to contact the Va. State Bureau of Statistics. Click here to visit their Vital Records web page.

Who do I contact concerning Patient/Treatment Records?

To request copies of any inpatient medical record, please mail a signed letter requesting copies of your records. Please provide the following information: name, address, sponsor's SS#, copy of ID (front and back), phone number, the year of the records you are requesting and the address to where the copies are to be mailed. Mail the request to: Naval Medical Center Portsmouth...620 John Paul Jones Circle...Portsmouth, VA 23708...attention Medical Records.

How do I submit a commendation/concern/suggestion?

If you are a patient and would like to commend or complain about the service that was provided to you, go to the Feedback Web page. Here you can pass comments to the Pharmacy, Customer Services, Safety, Housekeeping as well as feedback to the Webmaster if you notice dead links or erroneous information.

I am trying to locate someone at the command.  Can your help?

Normally we do not annouce email addresses or phone numbers for a specific individual staff member.  But, we will forward your email to the staff member, and he/she will respond if they desire to do so.  Staff privacy is respected at this command.

Do you have someone that will help watch my children while I'm in the hospital?

The Charette Health Care Center proudly offers a Child Waiting Center for families with medical appointments at Charette. Parents may now leave their children with Navy-certified child care providers during the period of their appointment! FREE! Located adjacent to the parking garage in Building 249 across from the ground floor entrance to the Charette Health Care Center. Open Monday - Friday, 0630 - 1630 validation of Appointment Required! Make your reservations today by calling (757) 953-6904.

Can I volunteer at Naval Medical Center Portsmouth?

The Red Cross is always looking for volunteers to be placed in the numerous wards, clinics and other areas of the medical center. You may obtain a Red Cross Volunteer application at the Red Cross office in Building 3 and at the Information Desk in Building 2, Charette Center, main hospital building. Please phone the Red Cross at 757-953-5435 or visit the Red Cross office for additional information, hours are Mon - Fri, 8 am - 4:30 pm.

How do I obtain past medical records?

All of our medical records are retired to St Louis Mo, every two to five years (every five years for outpatient records and every two years for inpatient records). You may contact them at the following address: National Personnel Records, 9700 Page Blvd, St Louis Mo 63132-5100 . Telephone 314-801-0800 , Fax 314 801-9195 . You will need to know what type of medical records that you are looking for (inpatient or outpatient) and from what hospital or clinic.

How do I send flowers to a patient?

The Flower Shop on the base will deliver flowers to our patients during their stay at NMCP. You may contact them at (757)397-5858 for more information.
