Travel and Leave Risk-Assessment Questionnaires

Description: Samples of travel and leave assessment questionnaires intended to provide an individual self-check and a command second check on a shipmates travel plans. In the case of the USS Vinson, the ship was doing a homeport shift that required a change from the West to the East Coast. This meant that much of the crew was relocating and driving cross-country. This form, like the others, is not scientific but intended engage the chain of command and making sure their sailor remained safe while driving long distances.
