FAQ For Service members (Page 2)
Why should I be a part of this process?
You are your own best health advocate. By completing the PDHRA you have an opportunity to take
charge of your health and take advantage of the health care and community services available to you.
Identifying health concerns early allows us to treat your health concerns before they become chronic
problems.
How do I file the DD Form 2900?
The form can only be completed in an electronic or web-enabled format. A PDF version of the form is provided here
as a preview of the questions that are included on the form. Service members will be contacted by
their unit leaders and given information on how to access the electronic or web-enabled version when
they fall within the timeframe for completing the PDHRA process. Information on how to complete the
form will also be in each military service's PDHRA implementation policy.
What can the PDHRA do for me?
The PDHRA empowers you to take control of your health. By identifying your health and readjustment
concerns early, you can resolve problems that could – if left unattended – negatively affect your
deployment status, career, or life beyond deployment.
How do I complete the initial PDHRA questionnaire?
Your unit will contact you when it is time to begin the PDHRA process. You will receive instruction
then about the next steps to take.
What happens after I complete the initial questionnaire?
A primary care provider will review your health concerns with you and discuss any deployment-related
health questions you may have. If needed, the health care provider will refer you for further
treatment or to additional resources.
