Enroll/Update for VA Medical Benefits
Enrolled veterans should use VA Form 10-10EZR to update or change their information such as income, address, phone number, change in health insurance, or next of kin information.
Unlike the 10-10EZ form, the 10-10EZR form CANNOT be submitted on-line. Click here for a fillable version of VA Form 10-10EZR. (You must have the latest version of Adobe Acrobat Reader on your computer to save or print the forms. Click here to obtain a free copy of the Adobe Acrobat Reader software.) You will need to complete, print and mail the form to your local VA facility for processing. This form must also be signed and dated. Click here for information as to who can sign the form. If the form is not signed and dated, it will be returned for completion.
