*NEW* Instructions for Online filing Proof of Claim

You may now file a Proof of Claim via an online form, if you are a creditor (the person who is owed money in a bankruptcy case) and are not represented by an attorney.

To do so, follow the link below, and search for the case against which you wish to file a claim. You may search by case number, or by the name of the debtor (the person or entity that has filed bankruptcy). When you find the correct case in the search results, click on the "Select this case" link to the right, and you will be taken to the online version of the Proof of Claim form. The case information will be pre-populated for you, and you need only provide your information, and information regarding the claim. For step-by-step instructions, click the "Show Instructions" link at the top of the form.

Please note: You will NOT be able to submit attachments and supporting documentation if you use the online Proof of Claim form. After you have submitted the form, you will be given instructions for sending this documentation directly to the Trustee for the case. In addition, if you include your email address in the space provided on the form, you will receive a copy of the completed Proof of Claim form by email for your records.

The online Proof of Claim application is designed for Internet Explorer. If you are using another browser, such as FireFox or Opera, you may find that some functions in the form do not work properly.

Instructions for Faxing Proof of Claim

You may now fax file a Proof of Claim to the court if you are a creditor (the person who is owed money in a bankruptcy case) and are not represented by an attorney (form below). 

Please complete the fillable form and fax it to the court at 602-682-4004. You should also include additional attachments supporting your claim such as invoices. Any personal and identifying information, such as social security numbers, financial account numbers, dates of birth, and names of minor children, should not be included.

The faxed claim will be held until reviewed by a court employee and, if accepted, will be considered filed as of the received date of the fax. A Proof of Claim cannot be accepted for filing if you do not reference the bankruptcy case number on the form. If you have received a pre-printed Proof of Claim form from the Court, please use it as it displays the debtor’s name as well as the case number.

Please contact the clerk's office at 602-682-4000 for additional assistance. Please include your telephone number on your Proof of Claim form should the court need to contact you. PLEASE DO NOT SUBMIT COVER SHEETS.

Please note: If you are a registered ECF user, and file your claim electronically, please refer to the Claims Filer instructions below to file your Proof of Claim.
