Welcome to the online application of the Cancer Prevention Fellowship Program!
Online applications must be submitted on or before 11:59 P.M. September 1, 2009 U.S. Eastern Daylight time.
In addition, supporting documentation (i.e. letters of reference, academic transcripts, other documentation) must be submitted on or before 11:59 P.M. September 8, 2009 U.S. Eastern Daylight time.
To be considered for the Program, you must meet the eligibility requirements and complete all parts of the application, as described below. If you have additional questions about the application process, please consult the Frequently Asked Questions

Eligibility Requirements
To be considered for the CPFP, you must meet the following eligibility requirements:

Doctoral degree.
You must possess an M.D., Ph.D., J.D., or other doctoral degree in a related discipline (e.g., epidemiology, biostatistics, ethics, philosophy, or the biomedical, nutritional, public health, social, or behavioral sciences). Foreign education must be comparable to that received in the United States.
Applicants currently enrolled in an accredited doctoral degree program that have not yet fulfilled all degree requirements will be considered for entry into the Program, with the understanding that all requirements will be completed before the start of the CPFP, June 21, 2010. Assurance to this effect must be supplied in writing by the chair of the dissertation committee (e.g., Ph.D. candidates) or the dean of the school (e.g., M.D. candidates). Please request letters of assurance through the application website.
Applicants must have less than five years of relevant postdoctoral training at the time of appointment.

Citizenship.
You must also be a citizen or permanent resident of the United States at the time of application (September 1). The I-551 stamp in a passport is acceptable; "Employment Authorization" documents are
not
acceptable.

Application Materials
The following application materials are required:

Personal Statement of Research Goals.
In narrative form describe your research interests and goals and how these relate to the field of cancer prevention and control. Please also provide insight into your short- and long-term career goals, and explain how the CPFP will help you in achieving those goals. Limit your personal statement to two typed, single-spaced pages and use 12-point font and 1 inch margins (approximately 1,000 words). Please upload your personal statement to your online application.

Curriculum Vitae.
Refer to the Information to Include in Curriculum Vitae section of our catalog. Please upload your
curriculum vitae
to your online application.

Letters of Reference.
Applicants must request current and original letters of reference from four individuals in the scientific/academic community who have knowledge of their scientific accomplishments, motivation, and skills. Please request letters of reference through the application website. If more than four letters are received, only the first four letters will be reviewed.

Academic Transcripts.
Applicants must provide transcripts (and/or translations, if applicable) from each institution where they received (or will receive) graduate and undergraduate degrees. Please scan and upload your UNOFFICIAL transcripts on the application website.

Other Documentation.
Permanent residents of the United States must submit proof of eligibility for citizenship. The I-551 stamp in a passport is acceptable; 'Employment Authorization' documents are not acceptable. Individuals applying through the Ireland-Northern Ireland-NCI Cancer Consortium
must submit proof of citizenship (birth certificate or passport). U.S. citizens are NOT required to submit proof of citizenship. Please scan and upload your citizenship documentation on the application website.

Online Application
The following steps are necessary to apply online:

In order to complete the online application, you must log in to an account that you previously created or create a new account.
Log in to an existing account.
If you created an account in a previous year, please log in to that account to create a new application for this year.
Create a new account.
If you have never created an account, you will first need to create an account before you can complete the online application. You will need to provide your name and a unique username, password, phone number, and e-mail address in order to create an account.

Complete and submit the online application.
Once your account is established, you may begin completing the online application. You will be requested to provide some general information and to upload your personal statement of research goals and
curriculum vitae.
You may complete the online application at your own pace and in multiple sessions, if you desire. Information entered online is saved as the application is completed and can be edited up until you submit the application. Once your online application is error-free, you may then submit it to the CPFP office by pressing the "submit" button. Your application must be submitted on or before 11:59 P.M., September 1, 2009 U.S. Eastern Daylight time. After you submit your online application, you will receive an e-mail confirming that it has been received. Except for updating your contact information, you will NOT be able to make any changes to your application
after it has been submitted
nor will any changes be accepted via electronic or postal mail.

Check that letters of reference, academic transcripts, and other documentation have been submitted.
You may log into your account to check to see what has been submitted. It is your responsibility to ensure that we have received your supporting documentation by September 8, 2009.
