- Contact your primary-care manager, medical department representative, or go to your healthcare provider as soon as possible.
- Be prepared to explain to the healthcare provider what happened, describe the illness or reaction, tell them the date and time it happened, and when the vaccination was administered.
- Request that your healthcare provider file a Vaccine Adverse Event Reporting System (VAERS) form.
- If you don’t believe your reaction is serious enough to visit a medical treatment facility, but you still wish to report it, you can contact VAERS yourself at 1.800.822.7967 or file a report at the following Web site: http://vaers.hhs.gov/
