Passport Assistance for Hoosiers

Air Travel
Effective January 23, 2008, ALL persons, including U.S. citizens, traveling by air between the United States and Canada, Mexico, Central and South America, the Caribbean, and Bermuda are required to present a valid passport, Air NEXUS card, or U.S. Coast Guard Merchant Mariner Document.

Effective January 31, 2008, oral declarations will no longer be accepted from U.S., Canadian or Bermudian travelers seeking to enter the U.S. by sea and/or land. Travelers will need to present a valid government issued ID and a birth certificate, naturalization certificate or baptismal record.

Travel by Land or Sea
Those departing from the United States on a cruise ship for a round trip in the Caribbean, are not required to travel with a passport at this time. If a U.S. citizen does not have a passport or a certified copy of a birth certificate, the following may be accepted as proof of citizenship: - A U.S. state or federal government-issued birth record (note: hospital-issued birth certificates are not acceptable) or baptismal record. If a U.S. citizen child was recently born, and no copy of the birth certificate is available, bring whatever paperwork the hospital has given you as a record of the birth. If you do not have a certified copy of your birth certificate, one can be requested from the Vital Records office in the State where you were born.

Please also check your destination country's entry requirements at http://travel.state.gov/travel/cis_pa_tw/cis/cis_1765.html. For example, even though a passport is no longer required for travel to Mexico, U.S. citizens must present proper identification upon arriving in the country.

Because of the volume of passport applications, Senator Lugar can only make inquires with the passport agency if the date of departure is less than 2 weeks away.  If your departure date is more than 14 days, please wait to contact us.

If you have not received your passport 48 hours prior to your departure, you can travel to the Chicago Passport Agency to reapply and receive a passport on site.  The Chicago Passport Agency is located in the Kluczynski Federal Building, 230 South Dearborn Street, 18th Floor, Chicago, IL 60604-1564. To download an application, get directions or obtain additional information, visit http://www.travel.state.gov.

If you are traveling within 48 hours and decide to travel to the Chicago Passport office, you will be starting the application process over.  Therefore, you will need all of the materials required when the application was first completed, including a certified birth certificate, photos and new application.

The Chicago office will process your application as a “walk in” if you are within two days of your travel date.  However, be certain to bring proof of your departure date (airline ticket, official itinerary from travel agent, etc.) and be prepared to stay all day.
