Application for Determination of Child Disability

Form AA-19a is completed by or on behalf of a child over age 18 who became permanently disabled before age 22 to determine entitlement to a benefit. Includes information on the child's medical condition, medical care, educational training, and work activity. To obtain a copy of Form AA-19a, please contact the nearest RRB field office.
