Perfusionist Licensing Program Advisory Committee Appointments

The Commissioner of DSHS requests your assistance in identifying applicants for membership on the Texas State Perfusionist Advisory Committee. The committee advises DSHS regarding the rules and operation of the Perfusionist Licensing Program.

The positions requiring appointments are for:

• Physician Member (physician member applicant must be a physician licensed by the Texas Medical Board who is certified in cardiovascular surgery)
• Two Public members

The term of these positions begins upon appointment and ends on January 31, 2015. We are currently authorized to reimburse members of the Texas State Perfusionist Advisory Committee for allowable expenses while on committee business.

All people who wish to be considered as a nominee must complete the Application below for Advisory Committee Appointment (Form AG-50). All applications will remain on file for twelve (12) months. Completed applications, including two letters of recommendation, should be mailed to the following address by April 2, 2009.
