1.Have submitted an application on the form prescribed by the State Board of Pharmacy.
2. Submit a check for $50.00 payable to the Oregon Board of Pharmacy. Checks are acceptable. However, all returned checks will be assessed a fee of $25.00 pursuant to ORS 30.701(5).
