Description: A Fluid Producer means a person who engages in the production of Grade A unpasteurized milk on a dairy farm and does not bottle the milk on the premises where production occurs, in pasteurized or unpasteurized form and for human consumption. This license is required to produce milk for sale to or use by a milk distributor.

Issuing a food license
A license application is presented only at the time approval is granted.
The department will perform routine periodic inspections of the facilities and collect samples of milk and water supply to ensure compliance with regulatory requirements. This license may be temporarily suspended for unsatisfactory results of inspections or sampling.

Prerequisites: Initial licensing: Prior to initial licensing the department will
    (a) review and approve plans for facilities and equipment
    (b) perform approval inspection of completed facilities
    (c) obtain samples of milk and water supply for regulatory testing.

The license will be issued upon satisfactory results from the above.
Application Form: Fluid Milk Producer License Application
Renewals: Annual license renewal is done by mail, with no inspection required. Renewal applications are mailed 45 days prior to expiration. The department will perform routine periodic inspections of the facilities and collect samples of milk and water supply to ensure compliance with regulatory requirements. This license may be temporarily suspended for unsatisfactory results of inspections or sampling.

NOTE: Penalty fees are assessed when a renewal license fee is 60 days overdue.
