Detailed Information for: Physical Therapist Licensure by Endorsement

Description: Physical therapists licensed in another state, but not licensed in the State of Oregon, may be licensed by endorsement if they meet with all the endorsement prerequisites.

"Physical therapy" means: (a) Examining, evaluating and testing for mechanical, physiological and developmental impairments, functional limitations and disabilities or other neuromusculoskeletal conditions in order to determine a physical therapy diagnosis or prognosis or a plan of physical therapy intervention and to assess the ongoing effects of physical therapy intervention; (b) Alleviating impairments and functional limitations by designing, implementing, administering and modifying physical therapy interventions; (c) Reducing the risk of injury, impairment, functional limitation and disability by physical therapy interventions that may include as a component the promotion and maintenance of health, fitness and quality of life in all age populations. (d) Consulting or providing educational services to a patient for the purposes of paragraphs (a), (b) and (c) of this subsection.

Fees - application, exam, etc.:  $150 initial endorsement application fee, plus the actual cost to the Board of conducting a nationwide background check, currently $40.00. Total Board application fee $190.00.

Examination Information: For purposes of endorsement, the following shall be considered the minimum passing score on the physical therapist or the physical therapist assistant examination provided by a Board-approved examination service
(a) For applicants examined February 1, 1996 and thereafter, the minimum overall passing score shall be based on a formula using the criterion-referenced scoring system verified by a Board-approved examination service;
(b) For applicants examined from January 5, 1983 through January 31, 1996, the minimum overall passing score shall be based on a formula using the national average raw score minus two average standard errors of measurement as determined by the Board;
(c) For applicants examined from January 1, 1976 through January 4, 1983, the minimum overall passing score shall be 1.5 standard deviation below the national average raw score verified by a Board-approved examination service;
(d) For applicants examined from January 1, 1961 through December 31, 1975, the passing of a written examination which in the opinion of the Board is substantially equivalent to the examination given by a Board-approved examination service;
(e) For applicants examined prior to January 1, 1961, the passing of an examination of the American Registry of Physical Therapists, or the passing of a written examination which in the opinion of the Board is substantially equivalent to the examination of the American Registry of Physical Therapists.
Continuing Ed Requirements: Effective 03/17/06 the PT Board adopted mandatory continuing education requirement rules. The following is a summary of division 35 rules: the certification period will be a 24-month period measured from April 1 through March 31 of each even numbered calendar year; the first certification period will actually be January 1, 2006 through March 31, 2008; the subsequent certification period will be from April 1, 2008 through March 31, 2010; a PT will have to complete 24 hours of continuing education during any 24-month certification period; a PTA will have to complete 12 hours of continuing education during any 24-month certification period; the Board will randomly audit PTs and PTAs upon renewal of their practice license in even numbered years; if randomly selected for audit, a Licensee will have to provide proof, to the Board, they have met the CE requirements for the period audited; failure to meet the CE requirements will be a violation, of Board Rule, and will be subject to Board discipline.

Prerequisites: (1) File a completed application form and pay a non-refundable endorsement application fee.
(2) Are at least 18 years of age.
(3) Are of good moral character as determined by the PT Board.
(4) Are graduates of an approved school for physical therapists as provided in OAR 848-010-0010 and 848-010-0015(5) Are currently licensed in any other state or territory of the United States.
(6) Are in good standing in every state where currently licensed and were in good standing at the time of lapse in every state where licensure has lapsed for any reason.
(7) Pass the Physical Therapist examination provided by a Board-approved examination service.

Renewals: (1) All physical therapist and physical therapist assistant licenses expire on March 31 of each calendar year, regardless of the initial issue date. Physical therapists and physical therapist assistants must annually renew their licenses to practice effective April 1 of each year. A license is considered lapsed if a completed renewal application is postmarked or received after March 31. A person whose license has lapsed must immediately stop practicing as a physical therapist or a physical therapist assistant and shall not practice until the license is renewed.
(2) During the first week in January of each year the Board mails a renewal application to each currently licensed physical therapist and physical therapist assistant at the licensee's mailing address on file with the Board.
(3) If the completed license renewal application is postmarked or actually received by the Board after March 31, the licensee is subject to a lapsed license renewal fee as provided in OAR 848-005-0020(1)(e) in addition to the annual license renewal fee.
(4) A licensed physical therapist must complete the renewal application form furnished by the Board and pay the annual renewal fee provided in OAR 848-005-0020(1)(c).
(5) A licensed physical therapist assistant must complete the renewal application form furnished by the Board and pay the annual renewal fee provided in OAR 848-005-0020(1)(d).

Required Documentation: As part of your application for an initial, renewed, or reinstated professional license issued by the Oregon Physical Therapist Licensing Board (PT Board), you are required to provide your Social Security Number to the PT Board. This is mandatory. The authority for this requirement is Oregon Laws 1997, Chapter 746, Section 117 (ORS 25.785) and 42 USC ý 666(a)(13). Failure to provide your Social Security Number will be a basis for the PT Board to refuse to issue, renew or reinstate the license. Although a number other than your Social Security Number appears on the face of the license, your Social Security Number will remain on file with the PT Board. If warranted, the record of your Social Security Number may be used for Child Support Enforcement, Federal and State Tax Administration, and the reporting of final disciplinary actions against your PT License to the Healthcare Integrity and Protection Data Bank (HIPDB). Any other use or disclosure of your Social Security Number will require your written authorization.
