Description: Pharmacists dispense and compound medications for prescriptions issued by physicians, dentists or other authorized medical practitioners.

Fees - application, exam, etc.:  There are different methods to being licensed in Oregon. If you are taking the Oregon North American Pharmacist Licensure Examination (NAPLEX) and Multistate Pharmacy Jurisprudence Examination (MPJE) the fees are:
Oregon NAPLEX Application Fee: $50.00
NABP NAPLEX Examination Fee: $430.00
NABP MPJE Examination Fee: $170.00.

If you have Score Transferred your NAPLEX from another state to Oregon, the Score Transfer Application Fee is $200.00

If you are applying for Transfer of your Pharmaceutic Licensure (Reciprocity) from another state to Oregon:
NABP Preliminary Application Fee $300.00
Official Reciprocity Application to Oregon: $200.00

Examination Information: The NAPLEX is the means by which the Oregon Board of Pharmacy assess the entry-level competence of a candidate for licensure. To receive a test score, you must have completed at least 162 of the 185 questions on the exam. Candidates completing less than 162 questions will NOT have their scores reported. It is in the candidate's best interest to answer all questions presented. The minimum acceptable passing score on the NAPLEX scale is 75. The passing score reported is NOT a percentage value.

To receive a test score for the MPJE, you must have completed at least 77 of the 90 questions on the exam. Candidates completing fewer than 77 questions will NOT have their scores reported. It is in the candidate's best interest to answer all questions presented. The minimum acceptable passing score on the MPJE scale is 75. The passing score reported is NOT a percentage value.

Candidates who have taken the NAPLEX and did not pass, and/or candidates who are seeking primary licensure in more than one state, must wait a minimum of 91 days to retake the exam.

Candidates who have taken the MPJE and did not pass must wait a minimum of 30 days to retake the examination.

The NAPLEX examination is valid for a period of 1 year from the date of the exam. The MPJE examination is valid for a period of 6 months from the date of the exam.

Examinations are given daily, Monday through Saturday (excluding holiday's) at various Prometric Testing Centers throughout the United States. Candidates may contact the Prometric Testing Centers only after they have received an Authorization to Test letter from the National Association of Boards of Pharmacy. Authorization to Test letters will be sent to candidates only after they have met all the Oregon Board of Pharmacy requirements.

Continuing Ed Requirements: (1) A pharmacist licensed must complete seven hours of continuing education in pain management as follows:

(a) A one-hour pain management course, specific to Oregon, provided by the Pain Management Commission of the Oregon Department of Human Services; and

(b) A minimum of six hours of continuing education in pain management. This requirement may be fulfilled by any combination of continuing education coursework focusing on pain management including but not limited to the treatment of terminally ill and dying patients, and those with chronic, non-malignant pain.

(2) A licensee must complete the required continuing education within 24 months of their first license renewal after January 2, 2006.

(3) A licensee must retain for three years, documentation showing they have met the requirement of this rule, and must provide this documentation if requested by the Board.

(4) The pain management continuing education required under this rule shall count towards the 1.5 continuing pharmacy education units required under OAR 855-021-0005, in the license cycle in which the pain management continuing education is completed. Any portion of this continuing education may count towards the requirement in OAR 855-021-0010(1)(a) for 11 hours continuing education in therapeutics.

Prerequisites: 1.Have submitted an application on the form prescribed by the State Board of Pharmacy.
2. Be at least 18 years old.
3. Be of good moral character and temperate habits.
4. Complete requirements for the first professional undergraduate degree as certified by a school or college of pharmacy which has been approved by the board.
5. Complete an internship or other program which has been approved by the board or demonstrate to the board's satisfaction experience in the practice of pharmacy which meets or exceeds the minimum internship requirements of the board.
6. Pass a National Computer-Based examination.

Required Documentation: 1. Complete the Oregon NAPLEX Application.

NOTE: The moral character statement on page 2 of the application must be written out in its entirety. All applicants are subject to a criminal background check.

NOTE: The Oregon NAPLEX Application must be notarized at the bottom of page 2.

2. Submit a check for $50.00 payable to the Oregon Board of Pharmacy. A personal check is acceptable. However, all returned checks will be assessed a fee of $25.00 pursuant to ORS 30.701(5).

3. Make a clear photocopy of your state-issued birth certificate. If you were born in a foreign country and do not have a birth certificate, we will accept a clear photocopy of your passport or naturalization papers. Please note that your birth certificate must be issued by your state, and not a copy issued by the hospital.

4. Submit a current photograph showing full front, head to shoulders. Photograph must not be smaller than 2" x 2". A passport size photograph works well.

5. Mail the Oregon application form, photocopy of your state-issued birth certification, photograph and check to the Oregon Board of Pharmacy at 800 NE Oregon Street, Suite 150, Portland OR 97232.

6. Have your university registrar's office submit to the Oregon Board of Pharmacy evidence of your pharmacy degree. Degree must state when your degree was conferred. A copy of your diploma is not acceptable.

7. Pharmacists who are currently licensed in another state or have allowed their license to lapse from the state where they were licensed by examination must also provide:
a. A list of where you have practiced since licensure. This is necessary in order to confirm you have been practicing at least one full year. List the pharmacy name, city and state; and
b. Have a letter from the state board where you practice verify your license is current and is in good standing.

8. Foreign pharmacist graduates must mail to the Oregon Board of Pharmacy the following items:
a. Your ORIGINAL Foreign Pharmacy Graduate Equivalency Certificate (FPGEC).
b. Test of English as a Foreign Language (TOEFL) examination score report.
c. Test of Spoken English - P (TSE-P) examination score report.
