Description: For workers' compensation claims in Oregon, a physician assistant must certify to the director of the Department of Consumer and Business Services, prior to treating a patient, that:
  • They are an Oregon licensed physician assistant

  • They have reviewed the packet of information


  • Return and submit the completed Physician Assistant's Statement of Certification form (Form 440-3650, which is located on the back page of the Physician Assistants' Guide to Oregon On-the-Job Injuries)

    Once you have submitted your statement form, you should review our List of certified care providers to ensure your information is correct.

    Examination Information: There is a self-test in the "Physician Assistants' Guide to Oregon On-the-Job Injuries".

    Prerequisites: 1. Must be an Oregon licensed physician assistant
