Issued when a vacancy has occurred at a nursing Home and an Oregon licensed Nursing Home Administrator (NHA) is not readily available. A provisional license is a temporary license that remains in effect for 100 days and may be renewed for an additional 100 days. Provisional licenses are specific to a nursing home, thus a new license is required if the employee secures employment at another facility. A provisional license may be issued to a qualifying employee who is knowledgeable about nursing home operations or to a person seeking endorsement/reciprocity from another state.
A provisional license is specific to the facility and is not portable.

Prerequisites: |
- Baccalaureate degree or higher from an accredited school of higher education (waived if applicant holds an NHA license issued prior to January 1, 1983).
- Experience with and knowledge of facility operations or current licensure in another state.
- Is of good moral character and in good physical and mental health.
- Criminal history free of felony convictions.
- Apply to the Board and remit required fee.
