Permit allows the permit holder to sell fireworks, retail fireworks, public display fireworks, and/or agricultural fireworks, including the purchase, possession, storage and transportation of fireworks when otherwise in conformance with applicable requirements of ORS 480.110 through 480.165, OAR Chapter 837, Division 12, and any other applicable federal, state and local laws, rules and regulations pertaining to fireworks. |

| Prerequisites: |
1. Application must be postmarked with a U.S. mail postmark or received no later than December 18, for a permit valid for the following year.
2. Application must be complete, along with a copy of a valid driver's license or photo ID of all authorized sales representatives and company officers.
3. Application must contain approval of local fire authority.
4. Application shall contain a corporate seal (if available), or signed by every partner if a partnership and be notarized. |

Renewal notices are sent out approximately 3 months prior to expiration date of original permit. |

Duplicates must be requested in writing, and include the signature of the person who originally applied for the permit. |
