Wholesale drug outlet means anyone engaged in wholesale distribution of drugs, including but not limited to manufacturers, repackers, own-label distributors' warehouses, chain drug warehouses, and wholesale drug warehouses, independent wholesale drug traders, and pharmacies that conduct wholesale distributions and their officers, agents, representatives and employees. |

| Required Documentation: |
1. Have submitted an application on the form and the fees as prescribed by the State Board of Pharmacy.
2. Application shall specify the name, full business address, and telephone number of the registrant.
3. All trade or business names used.
4. If required, Wholesaler controlled substance application.
5. If Out-of-State Wholesaler, Verification Form from Resident State.
Mail the application form and the required documentation to the Oregon Board of Pharmacy at 800 NE Oregon St, Suite 150, Portland, OR 97232.
NOTE: Change of ownership or location requires a new application, fee and registration within 15 days after change. |
