Examination Information: |
Exam Time: Upon receipt of competed application, schedule with Licensing Unit. Exam Locs: Portland, Salem, Eugene. Exam Pass: 75 percent or higher. Exam Repeat: After first failure: seven days; After second failure: 14 days; After third failure: 30 days; Any subsequent failures: 60 days upon scheduling with Licensing Unit. |
Bonding Requirements: |
Provide a Forest Labor Contractor Corporate Surety Bond in the amount of:$10,000 if no more than 20 employees$30,000 if 21 or more employees |
Insurance Requirements: |
Vehicle insurance, if transporting workers. |
Prerequisites: |
1. Sole Proprietor: Submit application, pay license fee, provide a Farm Labor Contractor Corporate Surety Bond, and provide Certificate of Insurance for vehicles. 2. Partnership: EACH partner must submit an application, pay a license fee, pay a Farm Labor Contractor Corporate Surety Bond,and provide Certificate of Insurance for vehicles. 3. Corporations and non-profit associations must also be licensed with the same requirements as in 1 and 2 above. Non-profits must show proof of their non-profit status. Licenses will be issued to the majority share holder for a corporation and in the association name for a non-profit. |
