Qualified applicants may earn certification in one or more of four individual fields of practice in cosmetology: Barbering, Esthetics, Hair Design, and Nail Technology.
As defined in ORS 690-005: "Cosmetology" means the art or science of beautifying and improving the skin, nails and hair. "Cosmetology" includes the study of cosmetics and their application.
Initial Application: $15 Examination: $25 per section Certification: $33 for two years per certificate for each field of practice. Renewal: $33 for two years per certificate for each field of practice. Late renewal fee, for each year expired: $25 Duplicate $5

Renewals:
Application for renewal shall be made in advance of the expiration date, which is the last day of the month two years from the date the license was issued.
Duplicate:
A duplicate, or replacement, license is issued only if the original document has been lost, stolen, disfigured or destroyed and if conditions outlined in OAR 817-035-0110 are met.
Required Documentation: Affidavit to certify that original license has been lost or destroyed.
Official transcript from a licensed cosmetology career school (in-state applicants) or Affidavit of Licensure (out-of-state applicants), photo identification, and Social Security card or written authorization/verification
