Description: For workers' compensation claims in Oregon, a nurse practitioner must become authorized prior to treating a patient. To become an authorized nurse practitioner, you MUST:

  • Be an Oregon licensed nurse practioner
  • Review the packet of information
  • Return and submit the completed Nurse Practitioner Statement form (Form 440-2882, which is located on the back page of the Nurse Practitioner's Guide to Oregon On-the-Job Injuries)
  • Receive your nurse practitioner authorization number

Once you have returned your statement form, and have received your authorization number, you should review our List of certified care providers to ensure your information is correct.
